Mayo Clinic partnered with IBM to leverage the Watson Clinical Trials Matching (CTM) solution to assist patient care and research teams with the complex process of identifying potential patient-to-trial matches. Watson CTM electronically compares unstructured text in the electronic health record data against eligibility criteria for all cancer clinical trials open at Mayo Clinic. The IBM Watson CTM solution is a web-based cognitive computing system which utilizes natural language processing, the same Watson DeepQA engine used to famously play Jeopardy in early 2011. 

Initially, Watson CTM was launched as a point-of-care solution for oncologists. Despite efficiently matching patients to trials, low provider adoption occurred due to multiple factors especially time constraints. The project team engaged Mayo Clinic management engineers/business consultants to assess, reengineer and integrate the process and workflow. Clinical Research Coordinators (CRCs) took on the role of screening patients with CTM. They generated a list of matching trials for each patient and provided them to the patient’s oncologist. The new process significantly increased trial enrollment and CRCs workload for patients on study.  The project team determined that the process needed to be further transformed by creating a dedicated trial matching team to balance workload and create a seamless experience.

The Watson CTM screening team has matured as the system covers new disease groups and clinical practices. As adoption increases, continued and valuable transformation is demonstrated through clinical trial accrual and advanced process capability and integration.  Patients have experienced benefits shown through an appreciable increase in clinical trial enrollments in the clinical practices that implemented the CTM product. In the 11 months after breast oncology implementation, there was approximately an 80 percent increase in enrollment to Mayo’s systemic therapy clinical trials for breast cancer. Recent metrics have shown an average monthly enrollment increase of 84%. Additionally, staff experienced significant benefits when comparing the CTM product process to the traditional manual method. In lung oncology, patient matches resulted in a more complete list of potentially eligible trials and were completed in less than 50% of the time compared to the traditional manual method.

Adoption and adaptation of a CTM product supported by a screening team enabled high volume patient screening for a large number of trials with agility, consumer centricity and greater awareness of clinical trial opportunities within the oncology practice. Mayo Clinic aims to increase patient recruitment into clinical trials without requiring additional provider effort which also helps manage physician burnout. Surveyed clinicians in gynecology agreed 100% that “The CTM system has helped to include and exclude trials prior to discussion with patients.” The speed and accuracy of a CTM product paired with a screening coordinator team allows physicians to efficiently and effectively develop treatment plans that reflect the full range of options available to support patient care.
