Cleveland Clinic is a multispecialty healthcare system founded in 1921. The organization’s mission is to provide better care of the sick, investigate their problems, and further educate those who serve. Our activities in support of our tripartite mission are centered on four care priorities: patients, caregivers (physicians and all employees), community, and the organization. Our vision is to become the best place to receive healthcare anywhere.
Nearly three million people worldwide suffer a type of heart attack called ST-Elevation Myocardial Infarction (STEMI). STEMI is caused by a complete blockage in one of the main coronary arteries providing blood to the heart. Without rapid treatment, the condition is fatal or leaves the patient with a permanent disability from a weakened heart. In 2014, Cleveland Clinic sought to improve the care of STEMI patients in Northeast Ohio. Cleveland Clinic implemented a four-step Comprehensive STEMI Protocol:
1) Emergency Department (ED) catheterization lab activation
2) STEMI Safe Handoff Checklist which clearly defines care roles and responsibilities
3) Immediate transfer to an immediately available catheterization lab
4) Radial artery access first approach to percutaneous coronary intervention
This program has led to an improvement in timely medication administration, the use of radial artery access (the preferred option), and time to treatment. Because of improvements in quality of care, the STEMI death rate has decreased by >50% and serious bleeding complications have also decreased by nearly 30% over five years of follow-up. High-risk subgroups of patients, including women and those experiencing the life threatening complication of cardiogenic shock, have shown particular improvement.

The Cleveland Clinic Comprehensive STEMI Protocol has received numerous important recognitions. A total of 21 scientific abstracts have been presented at national cardiology meetings highlighting the beneficial impact of the protocol. The relationship between operational improvement and reductions in mortality has been published in the American Heart Association’s medical journal Circulation Intervention The impact on improving STEMI care for women has been
(1) awarded a Young Investigator Finalist Award at a American College of Cardiology national meeting (one of the most prestigious awards of a scientific meeting),
(2) published in the highest impact cardiology medical journal in the world, and
(3) identified as the first study to show improvement in the care of women STEMI patients through operational improvement.
The Cleveland Clinic Comprehensive STEMI Protocol shows the power of collaboration of caregivers to drive healthcare operational improvement to save the lives of patients from the emergency life-threatening condition of ST-Elevation Myocardial Infarction. This project highlights the crucial role of operational excellence in supporting our vision to be the best place for healthcare anywhere and our mission of providing better care for the sick.
